Physical Fitness Profile Procedures
Did you know there are two types of profiles? One is the Air Force Form 469, Duty Limiting Condition Report, which covers the member's duty and mobility restrictions. The second is Air Force Form 422, Notification of Air Force Members Qualification Status, which outlines your fitness prescription. Follow these six steps to ensure you have the correct profile:
1. Schedule an appointment with your primary care manager.
2. Your PCM will initiate an Air Force Form 469.
3. Your squadron commander and first sergeant will get a copy of Air Force Form 469 via e-mail and will forward it to you.
4. You must notify your squadron unit fitness program manager to set up an appointment with the HAWC.
5. The Health and Wellness Center will generate an Air Force Form 422 listing all limitations for physical training testing.
6. Schedule a follow-up appointment with your PCM for any changes to your profile.
It is the member's responsibility to ensure their paperwork is current and correct. If you wait till the last minute, you could go overdue on your PT test.
